Diabetes Prevention Program (Prevent T2)

Why do you PreventT2? For the people and things that are important to you, the PreventT2 program can help you lower your risk of #type2diabetes.

Who can participate in the program?

  • Anyone at risk for Type 2 Diabetes
  • Anyone with Pre-diabetes
  • Am I at Risk for T2?

How does Prevent T2 work?

Prevent T2 groups meet for a year — weekly for the first 6 months, then once or twice a month for the second 6 months to maintain healthy lifestyle changes.

A lifestyle coach, specially trained to lead the program, will help you learn new skills, encourage you to set and meet goals, and keep you motivated. The coach will also facilitate discussions and help make the program fun and engaging. You will learn to:

  • Eat healthy without giving up the foods you love
  • Add physical activity to your life
  • Manage stress
  • Stay on track when eating out

Sign up today!

Contact your Lifestyle Coach, Kim Zientek, at kzientek@phmc.org or call 855-344-2844.
